Chasin Tails Dog Care Center agreement and waiver form.

Please note the following in order to keep a firm understanding between you--the client, and us:
I understand that I am solely responsible for any harm caused by my dog(s) to another dog while they are at Chasin Tails Dog Care Center. I understand that, if for any reason, Chasin Tails is unable to further care for my dog I must have an emergency contact available to pick-up and care for my dog.

I understand that I am to walk my dog(s) in and out of the facility on a leash, and in an expedient manner

I understand that dogs are not allowed to bark uncontrollably in the facility, and if necessary my dog will be muzzled for short periods of time as a training tool. I also understand that in order to maintain control and exhibit leadership in a large group of un-kenneled dogs certain humane training methods may be used as the owners or staff of Chasin Tails see fit given the circumstances.

I understand that my dog(s) must have a current City license.   I will be solely responsible for any and all fines levied if my dogs license is not valid.

I understand that check out time is noon at the latest for overnight stays and anytime after that is an added $21.50 for daycare. Please initial  & acknowledge. ________________

I understand that I must pay for care and services rendered in full at the end of my dog's stay at Chasin' Tails.

I understand that extra services such as administering medications or feeding my dog the food I have brought from home is subject to extra charges as so determined by Chasin' Tails.

I understand the dogs in Chasin' Tails care are fed once in the early evening and understand and accept the reasoning behind the schedule.

I have insured that my dog(s) are in proper health and are up to date with their shots and have been truthful with this information. I also know that I am solely responsible for the sickness caused to any other dogs from my lack of truthfulness.

I understand that dog lice is an issue in Calgary, and I am advised to use a preventative treatment monthly, (Revolution, Advantage, or Defend, as advised by Veterinarian) and at my own expense.

I understand that by bringing my dog(s) to an open concept care facility means that they will be playing and interacting with other dogs. Minor cuts or scratches are inevitable, and, though the dogs are carefully supervised at all times, and the staff and owners do their utmost to keep them safe, there is no guarantee that a more serious incident (scuffles, fighting, even death.) will not occur.

In case of an emergency, or sickness, as so determined by management or staff of Chasin Tails, if myself, or the emergency contact given is unreachable, I hereby give consent to Chasin Tails Management or staff to seek emergency care at the nearest Animal Hospital accessible.

I also understand that any medical, veterinary or other cost is my responsibility
